At that time, railroads and mines in America were some of the most dangerous places to work yet the workers had the least protection. Therefore the Safety First Movement was initiated to reduce workplace hazards.
The phrase has been used widely in the United States since then.
Used to describe a person being more careful after an accident or a bad experience in the past to avoid it happening again.
I drive more carefully after an accident when I was little because a burnt child dreads the fire.
