The phrase originated from the Safety First Movement in the 20th century.

At that time, railroads and mines in America were some of the most dangerous places to work yet the workers had the least protection. Therefore the Safety First Movement was initiated to reduce workplace hazards.

The phrase has been used widely in the United States since then.
